He is unstable and he is deeply, deeply mentally disturbed. Serial killers can resist the compunction to kill. But the fact is that his self-control is not perfect and he has a deeply engrained kill instinct.
As in Civil War where, when two bit villains walk into the room and he turns around and puts a bullet between their eyes as a reflex then tries to justify himself.
He might not fight the legal definition of insane in the sense that he would been deemed fit to see trial, like most serial killers. But he is definitively unstable.

If he was insane, he'd be trying to kill heroes by now.

Actually, I'm surprised I didn't remember this earlier. In "Purgatory" Frank dies but is brought back as an angel "of death" (so to speak), to kill villains/criminals and such. I'd imagine he wouldn't be selected by the big bearded man to do such if he was mentally flarked.

Then again, the angels thought that sending Frank back to Earth would be a punishment; when in fact he loves living in hell. So their judgement of him was shaky at best.

True, but they still selected him to carry out what he's pretty much been doing since the death of his family I like how he casually remarks on that during 'Welcome Back, Frank.'

If you read the script thingy at the end of the Welcome Back trade, Ennis only did that to refernce past cannon, and he wanted to get it over with and never mention it again (thankfully) as fast as possible.

That single line in WBF is the only reason I treat that event as canon LOL

I love how he absolutely owned Daredevil ;p You know, Ennis is horribly infamous for treating any hero other than Castle like shit, but that fight was actually very well done. It acknowledged Frank really can't take a majority in close-combat against Daredevil and relies on weaponry to stand a chance. I also liked the following scene involving the revolver and the sniper rifle

So angels, under the approval of the almighty himself, thought that sending Frank back to do what he loves most was a punishment? That is stupid on so many levels, most above all others it makes God look like an idiot.

Well, in that story Frank actually killed himself, obviously to finally put an end to his effort. Then they're like "NO WAI!"

Give death some mother fucking emotional meaning!

I think Omega Red is one of those guys that once he dies, requires the sacrifice ritual to come back. Just like his first appearance where a bunch of people were examining his body when he sucked the life out of them.

Just like his first appearance where a bunch of people were examining his body when he sucked the life out of them. I could see that happening. And yeah, they needed to do that since he was basically frozen for decades and requires the "life energy" to counter-act his carbonadium poisoning.
